Background: The orbit can be affected by many infections and contaminations, including preseptal cellulitis and orbital cellulitis. Preseptal cellulitis is a common infection of the eyelid and the soft tissue around the eye, which is located in front of the orbital septum and is diagnosed by the erythema and edema of the eyelid. Orbital cellulitis is an inflammatory process that involves the tissues located behind the orbital septum and inside the bony orbital cavity. Methods: The medical records of pediatric patients (aged between 1 month and 18 years) with preseptal cellulitis and orbital cellulitis admitted to Namazi Hospital from December 2107 to December 2023 were retrospectively reviewed. Results and Conclusion: A total of 189 patients, including 127 cases of preseptal cellulitis and 62 cases of orbital cellulitis were evaluated. The mean (± standard deviation) age was 5.9 ± 4.1years. The higher prevalence of orbital disease was winter and the highest incidence of preseptal cellulitis was reported in summer. In general, fever was the highest frequency of complaints in both disease groups. In physical examinations, swelling and redness of the eye have the highest frequency respectively. In the tests performed, the number of WBC, PMN and CRP was significantly higher in orbital cellulitis. Coagulase-negative staphylococci (CoNS) was detected in 3 cases. Rhinosinusitis was the most common radiological finding in both CT and MRI. Maxillary sinusitis was significantly more frequent in both groups of preseptal and orbital cellulitis patients. No need for surgical intervention in both groups of patients (preseptal and orbital cellulitis) and increased treatment in patients was reported.
