Abstract Introduction: Public health promotion can be achieved by effectively preventing congenital diseases, which demands accurate information about the prevalence and risk factors of these disorders. Therefore, the present study was conducted to ascertain the prevalence of congenital hypothyroidism (CH) and its risk factors among 500,000 neonates born in East Azerbaijan Province, Iran, within 10 years up to 2020. Methods: In this descriptive-analytical (cross-sectional) study, 500,000 live neonates born in East Azerbaijan Province (including all its cities) were enrolled using the census method. Venous blood samples were obtained from the heel and sent to a reference laboratory in Tabriz, Iran, to be screened for CH. Finally, the risk factors associated with CH were scrutinized using Poisson multivariate analysis. Results: The overall CH incidence was obtained at one per 135 live births. The CH incidence was 2.14 times higher in neonates born by cesarean section than in those born by natural delivery. Also, a family history of hypothyroidism (vs. the lack of a family history), admission to NICU, birth weight above 4000 g (vs. a birth weight of 4000 g), and preterm (vs. term) birth caused a 6.6, 4.6, 1.2, and 7.08 times higher risk of CH, respectively. Conclusion: CH had a much higher prevalence in the East Azerbaijan province of Iran than in other regions of the country.
